This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

At CCHC we strive to meet the healthcare needs of all patients, regardless of their insurance status or ability to pay. They can offer a wide variety of services through a Sliding Fee Scale.

The Sliding Fee Scale is available to all income-eligible uninsured patients, based on annual household income and family size.

Dental Hours:

 Monday, Wednesday, Thursday, 8:00 am - 6:30 pm.

 Tuesday, 8:00 am - 5:00 pm.

 Friday, 8:30 am - 6:00 pm.

 Saturday, 8:30 am - 5:00 pm.
