About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

NCHC has partnered with West Avenue Dental, located at 520 West Avenue in Norwalk, just a few blocks from the health center. Transportation assistance can be provided to and from the dental practice, and discounts are offered to health center patients. Children are welcome, Spanish speaking staff available.
The health center is not free. All patients are expected to pay something, unless covered by insurance. How much you will pay depends on your proven income and assets. NCHC has two full-time eligibility workers who will work with you to determine how much of a discount you will get at the center when you receive care.
Health centers are places where anyone can receive primary care even if they cannot afford to pay the full charge. This health center is also known as a FQHC – a Federally Qualified Health Center. This means that we are required to offer certain basic medical, dental, and mental health services to the residents of Norwalk in return for grant dollars from the federal and state governments. The health center must also comply with many federal and state standards designed to help ensure quality and access for all patients, without discrimination.
