About This Clinic

In keeping with the Gospel, Matthew 25 Health and Dental Clinic is dedicated to caring for people in need.
Matthew 25 is the only full-time clinic in Allen County offering free medical, dental, vision and hearing services all under one roof. The clinic asks patients for a $5.00 copay for a medical visit; $10.00 copay for a dental visit.
Our patients are uninsured and ineligible for Medicaid or Medicare.
All are low-income. Some homeless.
But the majority of them have jobs.
The fastest-growing group Matthew 25 serves is ages 40-60.
25 - 30 percent of Allen County residents are uninsured.
To qualify to become a patient
    A resident of Allen County
    Age 18 or older
    Have no dental insurance (including Medicaid Dental Benefits)
    Be at or below Matthew 25's income requirements
Matthew 25 DENTAL clinic will provide treatment for Veterans and patients that have Medicare (since Medicare does not provide dental coverage)
Your donations help to cover the costs of providing your care
    Please donate $5 for a medical visit
    Please donate $10 for a dental visit
To Become a dental patient...
You must come to a Walk-In Clinic on Monday, Wednesday or Thursday at 8:15 am. The Dental Clinic accepts 8 walk-ins, and we recommend you arrive early, approximately one hour, to be one of the 8 persons that will be taken. Emergencies will be seen that day, and non-emergencies will be determined on a case-by-case basis at that time.
Emergencies:  issue defined as pain, discomfort or swelling.
Non- Emergencies: Issues such as complete exams, cleanings, restorations, partials and dentures.
Walk-in clinics are subject to change depending on the availability of dentists. Bring:
    Photo Identification (drivers license, student ID, employee ID ..etc
    Proof of residence in Allen County (a current lease, utility bill, bank statement, car insurance….etc
    Proof of income
        1 month of continuous paycheck stubs for self, if married you must provide spouses income as well.
        Self employee-you must provide most recent tax return.
    A current list of any medications that you take.
You must have all the above documents with you at the time of making application. Applications cannot be processed without them.
