What to bring:
If you are a Medi-Cal patient, bring your BIC card and photo ID
Plan to spend a little time filling out paperwork if you are a new patient, so come 15 minutes early or download new patient forms (below) to fill out and bring with you.
If required, bring proof of income.
Uninsured/Cash Patients:
They offer a sliding scale fee schedule determined by your monthly income. This fee scale is determined by the National Poverty Guideline.
