Eligibility: Frail Elderly; All Ages with Permanent Disabilities; Medically Fragile Individuals (those who are not granted medical treatment until dental issues have been addressed); Income Guidelines Apply
Application: Application Available Online; Apply Via Mail, Fax, Website; NOTE:Complete entire application paying careful attention to Page Five that provides consent for the Program Coordinator to obtain and share information about you and also provides consent for your physician to release medical information.
