About This Clinic
Provides healthcare services to the homeless, including primary care,emergency dental care (extractions and temporary fillings), and mentalhealth services including dual diagnosis assessment and counseling atmultiple sites throughout Central and East San Diego County.Providesshort-term case management services to homeless patients of the clinic. Vision Care Services.Case management services include bus tokens formedical appointments, assistance in completing forms, and referrals foremergency food, clothing, social services, and obtaining an ID.Freetransportation to healthcare appointments at Family Health Centerclinics is provided to homeless patients, Mental Health, Dentalservices, and Case Management at selected sites only; coordinate withthe assigned case manager/social worker.
