This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Eligibility Assistance:
The trained Eligibility Coordinators will explain the insurance options for which you may be eligible, including Medicaid and expanded Medicaid coverage, Medicare and the new Marketplace. They will work with you to complete any necessary paperwork and help you identify any necessary documentation you need to gather to make the application process as quick and easy for you as possible.

If you don't have insurance:
Charter Oak Health Center offers a sliding fee scale for patients who don't have insurance. This scale is based on your income and family size. When you come for your visit, you will be asked to fill out an application and provide proof of your income, your address and your identification. You will be asked to pay the sliding fee amount that has been determined for you at the time of your visit.
