This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Payment:
FHCHC is a federally-qualified health center (FQHC) and provides care regardless of your ability to pay. 
They offer a sliding fee scale for patients that are not eligible for health insurance. Staff is available to discuss payment options if you cannot afford your care.

Dental Clinic Services:
- Exams
- Cleanings
- X-Rays
- Fluoride treatments
- Sealants (to prevent cavities)
- Fillings
- Extractions (if needed)

Fair Haven Community Health Center’s dental professionals are happy to offer a full range of preventative and restorative care to children and families.  As a Federally Qualified Health Center, all dental services are available to residents, regardless of their insurance status or ability to pay.

Accept Medicaid and private insurance, as well as a sliding-fee scale for uninsured patients.

If your child is seen on the mobile dental unit at one of the school-based health centers, they are encouraged to receive their regular dental care from FHCHC as well!
