- Sliding fee schedule for individuals without insurance ($50 minimum payment)
- Proof of identity and income required

Dental Office hours Monday, Wednesday & Friday: 9 a.m.Â-5 p.m. Tuesday and Thursday: 8 a.m.-4:30 p.m. First and third Saturdays of each month: 9 a.m.-1 p.m.
