When can I come to the clinic to register myself?
The clinic holds an open registration every Wednesday from 10AM - 1PM. Exceptions are made on a case-by-case basis if a prospective patient needs to register on a different day/time.
?Patients may not download or take paperwork ahead of time.
The Community Dental Clinic provides basic dental services (hygiene,
composites, extractions) to the uninsured adult community in upper
Pinellas county. Patients are qualified based on their income and their
location of residence.
To meet eligibility requirements of the
Community Dental Clinic, patients must fill out intake paperwork.
Appointments are offered every Wednesday from 10AM to 1pm to fill out new
patient intake paperwork. Prospective patients must bring all of the
following documents to intake appointments:
U.S. issued birth certificate, or U.S. passport, or Voters ID card AND:
Proof of current address (bill, rent receipt with your name on it)
Florida driver’s license or state ID card
Social security card
Once
a patient completes paperwork they are place on a wait list. When a
dentist is available, patients are called in the order of intake on a
first come, first serve basis.
Clinic costs: While the CDC is a free dental clinic, most patients make a charitable contribution toward the mission of the clinic at each appointment.
