The dental program promises to help your child achieve excellent oral health in an atmosphere of trust and mutual respect. Their program offers full dental services to children ages 3 to 20.

Required Documents
1. Your child’s Social Security Card and Insurance or Medicaid Card
2. Your Driver’s License or other form of photo ID
3. If you are not the parent, you will need to bring proof of guardianship or custody
