This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
Service programs provide general dental services for older adults ages 50 and older within the 300% Federal Poverty Guidelines.
Documents required:Photo ID, proof of income, and proof of residency for Collier County
Hours of operation Monday – Thursday from 8a-4pm.
Accepted insurance: Medicare and Medicaid. Sliding scale is available base on household income.
