About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Community Health Centers offers a program that reduces the cost of care for uninsured patients or can be applied to insurance deductibles and co-payments for patients with insurance. The discounts, based on family size and income, can be used for office visits and pharmacy prescriptions.

They accept most types of insurance plans, including Medicare, Medicaid, HMOs, PPOs, KidCare, Medikids, Healthy Kids, Children’s Medical Services (CMS) and other plans. Please call to ensure that your insurance is among those they accept. If you need financial assistance, please talk to the financial counselor if you need to make special arrangements.
