This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
FoundCare charges uninsured patients using a sliding fee scale based on family size and household income.
- Comprehensive dentistry includes exams, x-rays and treatment planning.
- preventive dentistry includes cleanings, fluoride and sealants.
- Restorative dentistry includes fillings and crowns.
- Emergency dental treatment includes toothaches, oral sores, and broken teeth.
Also accept the following insurance plans:
Dental: MCNA, DentaQuest, Delta Dental, Cigna, Metlife and Guardian.
New Health Center Hours:
8:00 AM - 5:00 PM - Monday, Wednesday, and Friday
8:00 AM - 7:00 PM - Tuesday and Thursday
Closed Saturday and Sunday
