Cash/Check (Check payments are processed instantly via ACH)
Sliding Fee Scale Program
FHCGA receives a portion of its funding from the US Department of Health & Human Services. This funding allows us to offer a Sliding Fee Scale Program to uninsured and under-insured patients. The chart below (base upon 2013 guidelines) is for informational purposes only and does not guarantee acceptance into the program and/or charges for services. All applications for the Sliding Fee Scale Program must be completed in person. Applications are accepted at the time of service.
