Thur: 8:00am-6:00pm
(closed every other Wednesday or Friday)

Grand Peaks Medical and Dental is a non-profit Community Health Center dedicated to providing quality primary and preventative healthcare as well as health related services to all members of the community.

Grand Peaks is a Federally Qualified Health Center (FQHC) that began as a small medical center in 2007, with one Provider and a small staff. 

Serve patients in St. Anthony, Rexburg and surrounding areas of the Upper Valley.

As a non-profit Community Health Center, the Grand Peaks organization is committed to providing affordable, quality medical and dental care to patients.

Whether you are insured, self-pay, uninsured, or low-income, offer affordable rates. 

These rates are discounted on a sliding fee scale based on proof of patient/family income and household size.
