About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

CCHC provides primary health and dental care to the uninsured, low income and homeless populations. Services include, but not limited to: immunizations, well-child visits, school physicals, prenatal care, general dental care, chronic disease management, and behavioral health care.

Mon, Tues, Thurs, Fri: 8am - 5pm
Wednesday: 10am - 7pm
Saturday: 9am - 1pm
Fees:Sliding scale fees. Medicaid, Medicare, private insurance, and private pay accepted. Uninsured patients pay a co-pay based on income.
