About This Clinic
This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
Provides preventive and restorative dental services to low-income/Medicaid eligible youth and adults, including routine cleaning, fillings, extractions, root canals, dentures, partials, bridges, crowns and other dental work.
Eligibility: Persons receiving Medicaid, Healthy Kids, MIChild, Healthy Michigan Plan, commercial insurance as well as those uninsured
Documents required: Proof of identification (drivers license or state ID) and proof of income to qualify for sliding fee scale
Fees: Sliding fee schedule available for those without dental insurance, based on household income
Hours:
Monday-Friday 8am-4:30pm
