About This Clinic

The Dental Program provides dental services for low income County residents who have no dental insurance. The following services are provided:
  Financial application process
  Dental office visits
  General dentistry
  Exams & x-rays
  Patient education
  Cleaning & fluoride
  Sealants
  Fillings
  Extractions
  Partial & full dentures
Eligibility:
*Low-income residents of Oakland County with no dental insurance or Medicaid
*If resident has no income, program asks that resident contact Department of Health and Human Services (DHHS) for financial assistance before completing application as an income is needed in order to determine applicant’s eligibility.
Application process: Call or go online for an application
Documents required:*Government issued photo identification
*Most recent pay stub, unemployment stub, letter of monetary determination or disability stub
*Most recent Federal Income Tax Return (pg. 1-2) with schedule C and/or E
The following documents if applicable:
Verification of monthly amount received from social security, Department of Health and Human Services, child support, retirement, pension, annuity or IRA.
Fees:
*$15 registration fee per family
*$15 visit fee per appointment
Hours: Monday-Friday 8:30am-5pm
