This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
Federally Qualified Health Center offering comprehensive medical, dental and ancillary services. Provides dental services for adults and children.
Hours: Momday, Tuesday, Wednesday, Thursday 8 AM - 7 PM;
Friday 8 AM - 5 PM
Intake procedure: Patient forms required
Fees:Accepts Medicaid/Soonercare/Medicare, Insurance, Sliding scale. minimum fee is $30 per basic service (exam, cleaning, filling, extractions) and $10 for x-rays.
Must meet income guidelines for sliding fee scale
Must provide proof of income to qualify for the sliding fee scale.
