About This Clinic
This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
Provides dental services to include: emergency and comprehensive exams, cleanings, x-rays, extractions, fillings, root canals and partials.
Hours: Monday, Tuesday, Wednesday, Thursday, Friday 8 AM - 5 PM
Appointment required
Fees:Accepts Medicaid/Soonercare, Sliding fee scale for uninsured, Insurance and Set fee.
Must provide proof of household income, list of all the members of your household, photo ID to apply for sliding scale
