About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Keystone Dental Care is a non-profit dental clinic founded in 1999 in Johnson City, Tennessee, with the mission of providing basic dental care to adults 19 years or older who meet federal poverty guidelines. We are funded by State grants, but also depend on private donations and local community support to keep the clinic operational.

How to Apply
Patients must be 19 years of age or older and must live in East Tennessee to qualify for treatment
     3 steps must be completed to apply to become a patient at Keystone Dental

1. Download application on our website.

2. Provide proof of income.


Hours: Mondays, Wednesdays, Fridays 8am to 4:30pm.

Tuesday and Friday hours vary.

3. Mail in application.

After your completed application is submitted with all required documentation, it will be reviewed and the information verified. When your application is approved you will be called and given an appointment time for a "New Patient Exam". At this time you will be informed of your co-payment amount.**
