This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
Low Cost/Sliding Fee Scale/Federally Qualified Health Center Dental Clinic
All patients at Servolution Health Services must go through pre-qualification screening. Patients must meet the following requirements.
cannot have insurance
must be at or below the 200% of the Poverty Level.
