ELIGIBILITY CRITERIA:
Total household income and benefits for all adults in the home
No medical insurance
May not be enrolled in other programs that duplicate services such as PATHS
Ineligible for government services such as:
Medicare B
Medicaid
Veterans Medical Benefits
Workers Compensation
Work Hour
Monday - Thursday 8:30 am - 5:00 pm
Tuesday evening 4:30 am - 8:30 pm
*by appointment only
Friday 8:30 am - 1:30pm
Saturday - Sunday Closed
