Why Should I Come?
Dental care remains a top health concern in our county.
Good dental health is important for your overall health including speech and nutrition.Untreated oral disease can be a risk factor for heart and lung disease, stroke, and diabetes.
Visit a dental provider soon, if you have any of the following issues:
    Broken teeth or fillings
    Roots of any of your teeth that are exposed
    Sores on the tongue, gums, or inside the cheek that do not heal
    Teeth that hurt when you drink hot, cold,or sweet beverages
    Bleeding Gums
Who is Eligible?
Our clinics serve children and adults who do not have access to a private dentist.
Acceptable dental health plans include:
    Medicaid and plans such as Dentaquest, Liberty Dental, and Argus Dental. Since dental plans may change, please call to see if we accept yours.
    Polk Health Care Plan. $10.00 copay for members.
    Low Income Pool (LIP) Grant. Contact our office to see if you qualify.
    Cash, personal check, or a credit card payment is accepted for select services if an individual does not have a dental plan as listed above. A $20.00 pre-pay is required.
Photo ID and proof of plan coverage (if applicable) are required at each visit. Children must have a legal guardian present at each visit.
What to bring to a dental appointment?
If this will be your first appointment in one of our dental clinics, we recommend that you arrive at least 15 minutes early to fill out the necessary paperwork.
Clients must present identification such as: Driver’s License, Social Security card, Medicaid Card, birth certificate, immunization records, or other official photo identification.
Parents or Guardians must accompany unemancipated children under 18 years of age for their first visit.
When & Where?
Our hours of operation are Monday through Friday from 8:00 AM to 5:00 PM, with the exception of holidays. Our office will close daily from 12:00 PM to 1:00 PM for lunch.
