About This Clinic
Eligibility:
(1) Live or work in Polk County for at least three months
(2) have no medical insurance
(3) have a family income that does not exceed 200 percent of the federal poverty guidelines.
 
 Fees: Free
Intake Procedure: Call for screening appointment 
 Languages Spoken: English, Spanish & Sign Language by request 
 Service Hours: 8am-12noon Monday 8am-5pm Tuesday - Friday 
 Description: Staffed almost entirely by volunteers, the clinic offers routine primary and dental care. Pediatrics, cardiology, gynecology, dental care, eye and ear care, vaccinations, mental health counseling, pharmacy and general medicine are examples of the types of treatment offered by the clinic. LVIM does not provide emergency services.
