About This Clinic
This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
I do not have any income, what will I have to pay?
Co-pays are based on sliding fee scale. You must provide a WG15 from the Georgia Department of Labor to verify your income. Based of the information provided on the WG15 your copay can be as low as $10.
Why is there a co-pay fee due each time we are seen at the clinic?
Any client who meets the guidelines as set forth by the (DHHS) Department Health and Human Services has a co pay due equal to there annual/monthly income as provided on the initial appointment and is updated every six months.
Dental clinic hours:
Monday & Tuesday 8:00 am – 7:00 pm, Wednesday & Thursday 8:00 am – 5:00 pm, Friday 8:00 am – 12:00 Noon.
