About This Clinic
The Dental Care Program is designed to assist those who are income eligible and who have no insurance or other methods with which to pay for needed dental work. Special funding set-aside for seniors and veterans.
hours of Operation:
Mon, Tue, Thu, 8:30 am - 5:00 pm.
Wed, 8:30 am - 6:00 pm.
Fri, 8:30 am - 4:00 pm.
Eligibility: Household Income must be at or below 125% of poverty level for the past 90 days; income limits are available on the McHenry Housing Authority website.
Maximum income guidelines: Household income must be at or below 125% of poverty level for the past 90 days for most clients. Seniors and veterans must be at or below 50% of area median income.
Documents needed: Photo ID, proof of income, proof of residency, Social Security card, and others per regulations.
