Dental (for TCHP) clinic patients to include: Diagnostic, Preventive and Restorative Services

Basic Wellness checks & Health Screenings
Lab Draws
Medication Assistance
TB Screening
Women’s Health

X-ray, Diagnostic Procedures and Limited Surgery
To qualify for care:
Resident of central Kane County or work in central Kane County for a minimum of 20 hours per week.
Unable to afford medical services
Have an annual income at or below 250% of the Federal Poverty Guidelines:
For example, it is $60,625 for a family of four, and $29,425 for an individual.
Not currently enrolled in an effective third party insurance. (Medicaid, Medicare, All Kids, Veteran’s Benefits,
disability or employer sponsored insurance)
What documentation is needed to qualify?
Proof of Residency or Employment in a town within central Kane County, i.e., utility bill, mail
Photo ID
Proof of Income:
Current Tax return
Current paycheck stubs or unemployment for past 30 days
Or… written statement from Employer if you are paid cash
