About This Clinic
This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
Provides primary oral health care for patients of all ages. General dentists are responsible for the diagnosis, treatment, management and overall coordination of specialized services related to the patient's oral health needs. Services generally include an oral health history, preventive dental cleanings, dental radiographs, oral cancer screening, an examination of the teeth to detect tooth decay or other problems, a periodontal examination to detect gum disease, dental restorations (e.g., fillings, crowns and implants), cosmetic dentistry, and other procedures such as fluoride treatment.

*Walk ins are welcome but there may be a wait time
Documents required:
*In order to receive services based on a sliding fee scale proof of income such as paystubs, benefit statements, unemployment insurance payments, or a recent tax return must be shown
*Government issued photo identification and proof of insurance is requested when available
Fees:
*Accepts Medicaid for children and adults
*Sliding fee scale based on income is available for the uninsured
*Flexible payment plans are available
*Most services involve some fees, and fees vary based on the service

*Monday, Tuesday, Wednesday and Thursday 8:30am - 5:00pm
*Friday 9:00am - 2:00pm
*Saturday 8:30am-Noon
*Closed Third Friday of the Month
