Fees:*Vary; if applying for sliding fee scale for those who do not have insurance bring proof of identification, two most recent pay stubs if working; proof of any other income
*Government issued photo identification, if you have dental insurance but no card, make sure that when you come in you have this information about the subscriber: social security number, full name and date of birth, bring all medication bottles
*For sliding fee scale bring proof of income (2 check stubs or bank statement, letter showing amount of unemployment benefits or SSI payment amount) and names/birth dates of all persons in household
*For uninsured, on site application assistance available for Medicaid and Healthy Michigan plan
hours:*Call for appointment or to find out hours for emergency walk in care (first come, first served)
*Taylor Center hours: Mon-Thu 9am-6pm, Fri 9am-5pm
*Lincoln Park: As of February 2017 Mon 9am-5pm, Wednesday 9am-5pm. Additional days to be added in Spring 2017.
