About This Clinic
Provides a variety of dental health services including preventive dental cleanings, dental radiographs, oral cancer screening, an examination of the teeth to detect tooth decay or other problems, a periodontal examination to detect gum disease, dental restorations (e.g., fillings, crowns and implants), cosmetic dentistry, and other procedures such as fluoride treatment.
Eligibility: Low income individuals and families
Application process: Call for telephone screening and appointment
Documents required:
*Must show government issued photo identification
*Insurance card if insured
*If requesting financial assistance must provide proof of income (income tax return or last three pay stubs or other income statements).
Fees: Based on ability to pay (requested minimum $20 for first visit)
