About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

When you need dental care, we're here for you and your family. Most PMS dental sites are co-located with a primary care health center. Our dental facilities offer emergency, preventive, basic restorative and a limited amount of rehabilitative or prosthetic dental care.

Everyone is eligible to receive PMS community health center services. We accept Medicaid, Medicare and private insurance. For the uninsured, a sliding fee scale is available based on income. Our health care guides provide on-site Medicaid or Health Insurance Marketplace enrollment assistance.
