About This Clinic
Provides coordinated access to medical and dental health care and support services to people living with chronic illnesses relating to HIV. Among the services provided are laboratory services, emergency housing, and free HIV antibody counseling and testing.
Call for phone intake. Answering machine operates after hours and on weekends. In 5-7 days a caseworker will contact client directly.
Eligibility
Must have diagnosis of any chronic disease, including HIV.
Documents required
Social Security card(s), photo ID/driver's license.
