About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Provides outpatient medical services including pediatrics and internal medicine, Dental, Optometry, Physical Therapy, Pharmacy with mail order, Laboratory, Radiology, Nutritional Services, Mammography, Health Promotion Disease Prevention, Public Health Nursing, WIC, Contract Care, Behavioral Health.
Intake procedure: Must complete application and create Chart
Fees: Accepts Medicaid, Medicare, Private Pay Insurances and offers sliding scale fees.
Eligibility: Must be member of a federally recognized Tribe. There are exceptions to this requirement. Contact agency for details.
Documents required: To open a chart and begin services must bring Social Security card; Certificate of Degree of Indian Blood (CDIB); Photo ID; or Letter of descendency issued by a federally recognized Tribe; Current insurance information (Medicaid, Medicare, SoonerCare or private insurance)
Hours: Monday - Friday 7 AM - 5:30 PM
