uninsured and underinsured patients with limited family income, offer patients a sliding-fee payment scale. Sliding-fee discounts are based on individual family size and income level. Hours:
Monday – Saturday, 8:00 am - 6:00 pm.
Wednesday, 8:00 am - 7:00 pm.
