This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

The sliding fee discount is confidential and persons applying for the discount must provide documentation of income, such as employee paycheck stubs, government benefits, etc.
Depending on qualifications, patients may be eligible for a 100%, 75%, 50%, or 25% discount on services.
Hours:
Monday-Friday 8:00 am to 5:00 pm.
