Thursday: 8:00am - 4:00pm
Neighborhood Health has board certified Dentists and Hygienists that treat adults and children. Our services include cleanings, x-rays, fillings and extractions.  We also provide referrals for those dental patients needing more specialized dental care.
Neighborhood Health is able to assist those with financial need through our sliding fee scale program. Funding for this program is provided by the Health Resources and Services Administration (HRSA) which is part of the US Dept. of Health and Human Services (HHS).  Through the sliding fee scale we are able to offer a discount on medical, dental, behavioral health and/or pharmacy fees to qualified individuals and families. Eligibility is based on family size and total family income.
?To qualify for our sliding fee scale call 703-535-5568 to schedule an appointment with one of our eligibility specialists. Prior to your appointment, please print out the application, complete and bring it to your eligibility appointment.  In addition to the application, bring documentation of all household income(s) to assist our Eligibility Specialists in determining your eligibility for the sliding fee scale. 
?Prior to scheduling appointments, new uninsured patients must meet with an eligibility specialist to determine their eligibility for the sliding fee scale.  See Sliding Fee Income Verification Form below.
If you are an existing sliding fee scale patient and your proof of income has expired or is about to expire, you must meet with an eligibility specialist prior to your appointment or your appointment may be cancelled.
Neighborhood Health’s sliding fee scale is the payer of last resort which means that if you have or are eligible for other insurance you are not eligible for our sliding fee.
