For uninsured medical and dental patients who qualify for sliding fee, instead of a percentage of total charges, you will be asked to pay a flat co-payment.
There are services that they provide that fall outside of the fee scale.
Patients will be responsible for any costs not covered. Upon request,
treatment plan with cost estimate will be provided. In order to be eligible for the sliding fee, patients MUST comply with all required forms, including a household assessment.
