About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Dental Clinic uses state of the art dental equipment to provide comprehensive oral healthcare while assisting with an appropriate and individualized treatment plan.

Dental Services:
 Dental hygiene
 General restorative oral care (fillings)
 Crowns and bridges
 Extractions, pre-molar and anterior root canals
 General dental examinations
 Emergency examinations, diagnostics, and treatment
 Implant crowns
 Limited orthodontic care
 Teeth whitening
 Radiographs
 Amalgam and resin restorations
 Complete, partial and implant dentures
 Extractions, including some third molars
 Adult and child prophylaxis
 Scaling and root planing
 Sealants
 Nutritional counseling

All CHCCW clinics offer a sliding fee scale to help you pay for the for the services you receive. The fee scale is based on income and family size and they are able to provide payment assistance because CHCCW is a Federally Qualified Health Center.

Hours:

 Monday - Saturday, 7:00 am. — 5:00 pm.
