About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Grand Peaks Medical and Dental is a non-profit Community Health Center dedicated to providing quality primary and preventative healthcare as well as health related services to all members of the community.

Since Grand Peaks is a non-profit Community Health Center, they receive some funding to help support our services and allow us to offer discounts.

All patients wishing to receive a discount must complete the Sliding Scale application and provide proof of household income.

Whether the patient is insured or not, if the patient qualifies for the Sliding Scale, the patient balance can be discounted.

The amount of discount depends on the sliding scale applied to the account after application is considered.
Grand Peaks is a Federally Qualified Health Center (FQHC) that began as a small medical center in 2007, with one Provider and a small staff.  The Dental center was added in 2010.  Have now grown to serve patients in St. Anthony, Rexburg and surrounding areas of the Upper Valley.
As a non-profit Community Health Center, the Grand Peaks organization is committed to providing affordable, quality medical and dental care to our patients. Whether you are insured, self-pay, uninsured, or low-income, we offer affordable rates.  These rates are discounted on a sliding fee scale based on proof of patient/family income and household size.
