Dental Services
Getting a check upWe provide a place for primary dental care for children and adults. Our dental home also offers integrated dental/medical care as we are a Community Health Center offering primary medical care for both children and adults.
Medicaid Dental Services
Adult Dental Coverage for Medicaid and expanded coverage for pregnant women have been reinstated as of July 1, 2014. Covered services include cleanings (for children), fillings, emergencies, and some extractions. Please contact the LCHD/CHC for questions and to make appointments for dental care for yourself or your children. Our knowledgeable staff will be happy to help you with all of your dental concerns.

Eligibility
    All Lake County residents
    We accept Medicaid and managed care as well as some commercial insurance.
    We do not accept HMO's or DMO's.
    Uninsured patients may apply for reduced fees for services based on their income. Services are not provided for free.
    Dental services covered by Medicaid are dependent upon your plan. Specific details may be obtained from your carrier.
    This is a general dental practice staffed by general dentists. Based upon need and complexity of case, patients may be referred for specialty care. Payment for specialty care will be the responsibility of the patient. The LCHD/CHC has no affiliation with the referral site or providers
